How Are Services Arranged?

When we receive a referral or a service request, whether it comes directly from the individual or an organisation, we meticulously document all the necessary details and promptly schedule a visit.

During this initial visit, our primary goal is to engage with the individual, their family, and/or designated representative.

This meeting serves as an opportunity to have an open and meaningful conversation about their desired outcomes and expectations.

Following our initial visit, whether it occurs during our first meeting or at a later scheduled encounter, we will engage in a comprehensive discussion with the individual, their family, and/or appointed representative.

This conversation will centre around the requested services and how they align with the anticipated outcomes. During this conversation, we will also address any potential risks associated with the services.

As part of our commitment to transparency and partnership, we provide the individual with a copy of their Needs Assessment, Care Plan, and Risk Assessment.

The Care Coordinator will proceed to carefully pair care workers with our individuals, ensuring that these care professionals possess the necessary skills, training, and experience to effectively meet their specific needs.

We also give due consideration to ethnic and language factors when making these assignments. In cases where nursing care is required, our dedicated nurses will be in attendance to provide the necessary services.

As we begin our service, a senior care worker or our dedicated care manager will be on hand to facilitate a warm introduction between the individual and their designated care worker(s).

This introduction serves as an opportunity for everyone involved to get to know each other better, discuss any specific preferences or needs, and establish a sense of trust and rapport.

We understand that the initial stages of care can be a significant adjustment for both the individual and their care worker. Our aim is to make this process as smooth as possible, ensuring that everyone feels at ease and confident in the care relationship that is about to commence.
